Kay Wing PT, DPT, NCS, GCS

Founder and President

Kay Wing, founder and president, received her Doctorate of Physical Therapy from Northern Arizona University and her certificate in physical therapy from Northwestern University. During her 35 years of practice she has managed rehabilitation departments throughout Phoenix.

Kay is currently president of the Arizona Physical Therapy Association. She is certified by the American Physical Therapy Association as a Neurological Certified Specialist, as well as a Geriatric Certified Specialist. She is an adjunct faculty for Northern Arizona University and for A.T. Still University. In 2008, Kay received the Clinical Excellence in Neurology Award from the Neurology Section of the American Physical Therapy Association. In 2003, the Arizona Physical Therapy Association awarded her the prestigious "Physical Therapist of the Year" award. Her passion is to work with survivors of stroke and other neurological injuries.

She enjoys running, sailing, hiking, biking and reading.


Ericka BallardEricka Ballard, Billing Representative

Ericka is from Northern California, and has been in Arizona for 3 years. Currently she is studying at Grand Canyon University to pursue a degree in Psychology and Social Work with the determination to become a doctor of psychology. Not only does she bring with her a vast background in Healthcare and Insurance, but she also brings a mean serve while playing her favorite sport tennis.


Warren Lake, PT, DPT

Physical Therapist

Warren is a Utah transplant, and received his Doctorate of Physical Therapy degree from the University of Utah. His education has included several published abstracts and manuscripts on movement disorders rehabilitation and falls. He has also collaborated with the National Institutes of Health in Washington DC. Outside of his work Warren enjoys fly fishing, and playing with his 2 children, an active red head, and her blonde cherubic counterpart.


Monique Macias, OTR/L

Occupational Therapist

Monique graduated from Midwestern University with her occupational therapy degree. She received the ArizOTA COTA Of The Year award in 2002. Among her neurological rehabilitation experience is field work in adult neuro rehab and stroke rehab at the University of Birmingham Hospital, home of the Constraint Induced Movement Therapy research program. At Scottsdale Healthcare Monique worked in the acute neuro rehab program and was on the development team for the intensive outpatient neuro rehab day program. When Monique isn't hands on remodeling her home she enjoys reading and hiking.


Liz Meisenheimer, PT

Physical Therapist

Liz is a Phoenix native and received her physical therapy degree from California State University, Fresno in 1979. She has had a variety of experiences in neurological and orthopedic inpatient and outpatient settings. In the past 2 years she has specialized in outpatient neuro rehab. Liz is currently serving as the Clinical Director at SWAN. She enjoys spending time with her husband and three daughters in addition to gardening, cooking and scrapbooking.


Nathan Miller, PT, DPT

Physical Therapist

Nathan received his Doctorate of Physical Therapy degree from Northern Arizona University in 2008. While volunteering during a week long neurological summer camp, he found a passion for working with patients who have survived stroke and brain injury. Nathan was excited to join the SWAN staff in July, 2008. Outside of work Nathan enjoys martial arts, specifically Tai Chi and Kung Fu along with fly fishing and snow boarding.


Mary RossiMary Rossi, Office Manager

Mary was born and raised in New Jersey and has recently moved to Arizona from West Jordan, Utah with her husband Edward. She is a LPN and Certified Coder and has worked in the medical field for many years. On her time off, Mary enjoys fishing, hiking, reading, and traveling.

Physical Therapist Assistant

Anar received her associates of applied science degree in 2005 for Physical Therapist Assistant from Gateway Community College. She has been with SWAN Rehab since March 2006. She has extensive training on REO Therapy. Anar, an avid hiker, is fluent in Gujarti an Indian dialect and hails from Detroit, Michigan.


Leonard Wood, PT, DPT

Physical Therapist

Leonard was born in Virginia, grew up in Maryland. He now lives in sunny Arizona with his wife. He recently earned his Doctorate of Physical Therapy from Northern Arizona University. He joined the SWAN team in June 2007. He discovered his passion for physical therapy after the many years he spent as a professional modern dancer. Leonard also enjoys gardening: dahlias are currently his favorite bloom. He loves hiking and many different genres of music.


Emily Roberts

Patient Care Coordinator

Emily is an Arizona native and received her degree in journalism from Walter Cronkite's School of Journalism and Mass Communication at Arizona State University in 2008. She joined the SWAN team in October 2009. When she's not working, Emily enjoys traveling, going to concerts and spending time with her dog, Patch.
