Evaluation and Scheduling
- A potential patient must have an evaluation before beginning therapy so that an individual program can be designed. For those patients living outside of Arizona, special arrangements can be made for the evaluation on the frist day of therapy.
- SWAN sends progress reports to referring physicians and has ongoing communication with them.
- Families are encouraged to be involved and may observe and participate.
Scheduling/Payment
Payment for SWAN's services may be covered in whole or in part by insurance. SWAN Rehab will submit claims to insurance companies to obtain reimbursement. Unpaid claim amounts are the responsibility of the patient.
